Scrubs vs Lab Coats: When Each Is Appropriate
Scrubs and lab coats solve different problems, and the convention for which to wear where varies by institution and specialty.
What each is for
Scrubs are the garment itself — you wear nothing underneath them clinically. They are designed to be changed at the start and end of a shift, laundered hot, and to carry nothing from outside the clinical area in.
Lab coats are a layer worn over other clothing. They protect what is underneath from splash and contamination, and they signal role. The traditional white coat also carries considerable professional symbolism.
Where each is expected
- Operating theatres, procedure rooms, labour wards — scrubs, usually institution-supplied, often in a theatre-specific colour, and generally not to be worn outside the department.
- General wards — scrubs are now common for nursing and increasingly for medical staff.
- Outpatient clinics and consultation — varies widely. Some consultants wear a lab coat over ordinary clothing; some wear scrubs; many wear neither.
- Laboratories — lab coats, for chemical and biological splash protection rather than clinical convention.
The bare-below-the-elbow question
Many healthcare systems introduced bare-below-the-elbow policies on infection-control grounds — no long sleeves, no wrist watches, no rings other than a plain band during direct patient care. Where such a policy applies, a long-sleeved lab coat is removed or rolled for patient contact, which limits how useful it is on a ward round.

Wearing both
A lab coat over scrubs is common for doctors moving between wards and consultation, giving pocket capacity and a visible role marker over the working garment. In that combination the coat is the outer layer and comes off for hands-on care.
Practical selection
For lab coats, the things that matter are length (consultation vs full laboratory length), pocket placement, and fabric weight — a heavy drill coat over scrubs in Singapore's humidity is punishing, which is why lighter stretch coats have largely replaced them. See lab coats.
